When Additional Support Is Needed
Health Services at Fair Haven provide clinical care and rehabilitation within a coordinated Life Plan Community. Whether care is needed for short-term recovery or ongoing medical oversight, services are delivered by licensed professionals in a setting designed for continuity and coordination. Because these services operate within one established campus, care teams coordinate closely when higher levels of support are required.
Rehabilitation Services
Rehabilitation Services support individuals recovering from surgery, illness or injury. Fair Haven has a separate, dedicated rehabilitation building for rehab guests, which includes a therapy gym, on-site pharmacy and spaces designed to support focused recovery.
Licensed therapists provide physical, occupational and speech therapy, working alongside nursing staff who deliver comprehensive clinical care throughout the rehabilitation process. Short-term rehabilitation helps individuals regain strength and function while remaining within Fair Haven’s coordinated system of care.

Long Term Care
Long Term Care at Fair Haven provides 24-hour nursing care for individuals requiring consistent medical oversight. Care plans are developed in collaboration with physicians and medical providers to ensure attentive, individualized care.
Services are appropriate for both long-term residency or transitional care following rehabilitation. Advanced memory care for individuals with more complex cognitive needs is available within the long-term care setting.

Care That Works Together
Health Services at Fair Haven are integrated within the Life Plan Community model. Communication between residential living, nursing and rehabilitation teams supports continuity when transitions of care are required. A Life Plan Community improves care coordination and supports seamless transitions of care within the same community.
