Long Term Care offers 24-hour licensed care for individuals who require ongoing medical management, advanced memory support or long-term assistance.
A Household Approach To Long Term Care
Fair Haven’s Nursing Care operates within the Household Model.
Residents live in smaller long term care households neighborhoods designed as residential settings while supporting complex care needs. Living areas are personalized, and private rooms are available.
Residents are able to maintain familiar routines for meals, rest and daily activities while receiving attentive care throughout the day and night. Because care teams work closely with the same residents over time, they are able to recognize changes in health and respond quickly.

Long Term Care at Fair Haven combines licensed oversight with daily support. Services are delivered within the household model so medical care and personal assistance work together in a cohesive manner.

A Team You Can Rely On
Medical care is overseen by an experienced medical director. Nursing services are delivered by registered nurses, licensed practical nurses and certified nursing assistants working in close collaboration. Many caregivers have served at Fair Haven for a decade or more, contributing to continuity and a clear understanding of residents’ evolving needs.
Care plans are developed in collaboration with physicians, therapists and families to ensure alignment and clarity.
Support for Advanced Alzheimer's and Dementia
For individuals living with advanced Alzheimer’s disease or other forms of dementia, Fair Haven provides a secured neighborhood with nursing support. This level of care is designed for residents whose cognitive needs and nursing require closer supervision and coordinated medical oversight than Assisted Living Memory Care provides.
